Cost: $200 Non-Refundable 

You will need to provide:
Birth Certificate, if you haven't been baptized
Baptism Certificate
First Communion Certificate - if received
Proof of Payment
If the student has legally changed their name, the legal identification
*The student needs to provide the sponsor's sacramental certificates 
 (Baptism, First Communion & Confirmation) At the time of registration!
