First Comunion
Please read the instructions carefully.
It is a requirement that the Parents have their own email
The student understands, reads, and writes English to be in the program.
To fill out this application, you need to present proof of payment and the Baptism certificate, if the student also needs the Baptism, you need the birth certificate.
You must complete one form for each student you are registering and it must be completed by a parent or legal guardian.
The student will not be registered if we have not received the baptismal certificate/birth certificate and full payment for the year.
The Baptism will take place in the second year after Easter and before the First Communion. Parents and Godparents must have attended the Baptismal instruction prior to the Baptism date.
**NOTE: For Baptism -- Godparents need to provide proof of their Sacraments -Baptism, First Communion & have attended the Baptismal instruction
Confirmation Sponsor must provide proof of receiving her/his sacraments - Baptism, First Communion & Confirmation.
. If one or all of the Sacraments are missing, the godparents can sign up for our RCIA (English) or RCIA (Spanish) Adult Sacraments program.

Complete the application according to the Student's school grade on September 2023.
